    Default affidavit of cohabitant form

Hello I just join the site. I am applying for a firearm and I took the affidavit of co-habitant form with my mum signature to the notary.
The notary said he needed my mum to be there as well. Do you know if this is true.? I don't see anywhere on the form indicating the cohabitant has to be there for the notary. I don't see any info on the websites indicating this either.

    Thank You
    K

That would make sense to me that you would both need to be there since she’s giving you permission to possess a firearm on her property/home.

I am a notary. The person signing the affidavit has to show identification and sign in the presence of the notary.
